Phillis Stokes, Bronx Delegate at LargeHello I’m Phillis Stokes, Bronx Delegate at Large and your Workman’s Compensation Liaison. Part of my responsibilities is to keep the membership inform of what steps is necessary when a member becomes sick or injured on the job.

You must follow these steps so that you will receive the correct benefits under our contract and the Workers’ Compensation Law.

1 . Report your accident to a supervisor. Thc supervisor will fill out an accident report (sometimes called an incident rcport, a C-2, or a C-2 package). The – supervisor should also give you Form DP-2002.

2. Fill out Form DP-2002. You should choose Option 1, which means you will use your sick andlor annual leave.

3. Fill out a rwuest for Sick Leave Grant. When you run out of accrued time, the agency head may grant you additional paid leave, but you need to make a request for that grant. If your grant request is approved, you can be paid a salary for up to three months or up to eighteen months if you were assaulted.

4. CalI Grev & Grev. LLP. They will send you a C-3 form to file with the State Workers’ Compensation Board and a C-4 form to bring to your doctor. You must file these forms to protect your right to workers’ compensation benefits, including time restoral, medical treatment, and workers compensation payments.
